Why the buccal fat pad resists weight loss
The buccal fat pad is a structurally encapsulated mass of specialised adipose tissue. Unlike general subcutaneous fat — which shrinks proportionally with overall weight loss — the buccal pad is relatively stable in size across weight changes. After major weight loss (15–30+ kg), patients often see significant slimming everywhere except the mid-cheek.
Timing matters significantly
For post-weight-loss patients, two timing considerations are critical:
- Weight stability. Wait until your weight has been stable for at least 6 months before considering buccal fat removal. Otherwise, further weight loss may produce hollowing on top of the surgical hollowing.
- Age-related volume loss. Major weight loss accelerates the visible appearance of facial ageing because subcutaneous fat is lost. If you are post-weight-loss and over 40, the combination of factors may make you a poor candidate for buccal fat removal.
Cautions for post-bariatric patients
Patients who have lost significant weight through bariatric surgery have additional considerations:
- Skin laxity from weight loss may persist in the face — buccal fat removal can worsen the appearance if the skin is too lax to redrape
- Nutritional deficiencies (common after bariatric surgery) must be addressed before any elective surgery
- Some bariatric patients regain weight over time; if your weight is still trending, wait for stability
For some post-weight-loss patients, the better answer is fat grafting or filler to restore lost volume — the opposite of buccal fat removal. A thorough assessment is essential.
