Conservative partial removal

This is Doç. Dr. Erdal's preferred approach for nearly every buccal fat removal he performs. The technique itself is the same standard intraoral approach — but the philosophy, the volume removed, and the patient selection are different from clinics that market 'maximum chiselling' or 'snatched cheek' results.

The philosophy

The buccal fat pad evolved to do specific anatomical jobs. Removing all of it for aesthetic effect borrows volume that the patient may need back later — and the loan cannot be repaid.

Conservative partial removal recognises three realities:

  1. Aesthetic trends change. The "Hollywood cheek" look that flatters in 2026 may not be the look of 2036 or 2046. Conservative results age into any aesthetic; aggressive results are locked into the moment they were performed.
  2. Faces age toward thinness. Natural age-related fat loss continues for the rest of your life. Starting from a thinner mid-face means you reach a hollow appearance years earlier than you otherwise would.
  3. Reversibility matters. You can always come back for more fat removal in a second procedure. You cannot come back for fat replacement (fat grafting partially restores volume but not the anatomical pad). One-way doors deserve more caution than two-way doors.

The volumes

An average adult buccal fat pad body contains approximately 9–10 grams of fat. Aesthetic removal volumes vary widely between surgeons:

  • 1–2 grams per side — very conservative. Subtle, lasting refinement. Recommended for borderline candidates or those near the upper age limit (mid-30s).
  • 2–3 grams per side — Dr. Erdal's standard volume for well-selected candidates. Visible refinement with strong long-term aesthetic outcomes.
  • 3–4 grams per side — aggressive. Visible immediately but ages prematurely. Rarely Dr. Erdal's choice.
  • 5+ grams per side — maximum/total resection. Strongly discouraged. Produces hollow appearance that worsens with age and cannot be reversed.

Why patient selection comes first

The same 2–3 gram resection produces:

  • A beautiful refinement in a 28-year-old with full, round cheeks
  • An over-thinning in a 42-year-old with already mature facial volume distribution
  • A barely-noticeable change in someone whose fullness was actually subcutaneous fat, not buccal fat

This is why Dr. Erdal's consultation begins with assessment, not measurement. The volume is chosen after determining the patient is a candidate, not before. Patients who insist on a specific volume removal in advance of consultation are politely redirected to a less results-focused consultation conversation.

How conservative results age

Aging trajectory comparison

Conservative partial removal leaves enough buccal fat that:

  • The cheek does not look meaningfully different at age 60 than at age 50 — the loss is gradual, blended with normal facial ageing
  • If additional fat loss occurs naturally, the patient still has cheek volume to lose without becoming gaunt
  • If aesthetic preferences change, the modest result is easier to live with than an aggressive one
  • Revision (if ever needed) is to add volume (filler, fat grafting) — a much easier, more reversible intervention than the original surgery

Frequently asked questions

Will the result be visible enough to justify the surgery?

Yes. Conservative partial removal still produces a clearly visible refinement at 3–6 months. The change is more 'photo-recognisable than dramatic' — the kind of change friends notice as 'you look refreshed' rather than 'you had surgery'.

Can I have more removed later if I want?

Yes — revision to remove additional fat is possible, though Dr. Erdal generally discourages it. If the result feels too subtle, an additional intervention (e.g. masseter Botox) is usually a better-considered next step than a second buccal fat resection.

Is conservative removal cheaper than aggressive?

No — the cost is the same. The difference is in approach, not in time or instruments.
