What 'minimal-incision' actually means
In buccal fat removal, all incisions are inside the mouth and hidden from view. The standard incision is approximately 1 cm. "Minimal-incision" usually refers to variations using 5–7 mm incisions, sometimes combined with specialised instruments to retrieve the fat through a smaller opening.
From a patient-facing perspective, the difference is invisible. From a surgical perspective:
- A 1 cm incision and a 5 mm incision both heal within 1–2 weeks and both leave no visible scar.
- The smaller incision restricts the surgeon's visualisation, making the procedure more difficult and slightly increasing complication risk.
- The smaller incision can make controlled resection harder, slightly increasing the risk of over-resection from blind pulling of fat through a tight opening.
When smaller incisions are genuinely useful
There are specific situations where slightly smaller incisions can make sense:
- Combined procedures. If buccal fat removal is added to a longer procedure (e.g. rhinoplasty), a smaller incision shortens the procedure slightly.
- Asymmetry correction. When only a small volume needs to be removed from one side, a smaller incision matches the smaller volume being retrieved.
- Patient anxiety. Some patients are reassured by the idea of a smaller incision, even if the clinical difference is negligible.
None of these are reasons to seek out a "minimal-incision specialist" — most experienced surgeons adjust incision size to the operation at hand.
What you should optimise for instead
Incision size is not the right variable to focus on. The variables that actually determine your outcome are:
- Patient selection. Are you a good candidate at all?
- Volume of fat removed. Conservative versus aggressive resection. This is the single biggest determinant of long-term outcome.
- Anatomical region targeted. Removing fat from the body only, or also from the extensions (which we recommend against).
- Surgeon experience and judgement. Especially around when not to operate.
A surgeon obsessed with incision size while indifferent to these other variables is the wrong choice. A surgeon who uses the standard incision but selects patients carefully and resects conservatively is the right choice.
