What each procedure does (and doesn't do)
Buccal fat removal targets a specific encapsulated fat pad in the mid-cheek. It refines the cheek hollow below the cheekbone and slightly accentuates cheekbone definition. It does not affect the jaw line or the lower border of the face.
Masseter Botox reduces the bulk of the masseter muscle — the chewing muscle at the angle of the jaw. Over 4–8 weeks the muscle thins, and with it the visible width of the lower face. It does not affect the mid-cheek volume.
If you look at a fuller face in three-quarter view, the two fullness contributors are usually obvious: a mid-cheek "bulge" (the buccal fat pad) and a lower-jaw "squareness" (the masseter muscle). Each procedure addresses exactly one of these. Together they address both.
Why the combination is so common
The combination is favoured by experienced surgeons and refined patients because:
- Synergistic visual effect. A subtle 2-gram buccal fat removal combined with 30-unit masseter Botox produces a more visible aesthetic change than 4 grams of buccal fat alone — without the long-term risks of aggressive resection.
- Single visit. Both procedures can be performed in one Istanbul visit. Masseter Botox is administered the same week as buccal fat surgery, often the same day.
- Low risk profile. Masseter Botox is reversible — if you don't like the result, it wears off in 3–6 months and you can decide whether to repeat or not. The two combine without compounding the surgical risk profile.
- Maintenance. Masseter Botox is a 6-monthly maintenance treatment, allowing you to "dial in" the look over time rather than committing to a permanent surgical outcome.
The combined treatment plan
- Day 0: Buccal fat removal surgery. Masseter Botox can be administered the same day or 7–14 days later.
- Week 2–3: Buccal fat swelling resolved. Masseter Botox starts to take effect.
- Week 4–6: Full masseter Botox effect. Combined visual change becomes clear.
- Month 3: Buccal fat result at ~95%, masseter Botox at peak.
- Month 4–6: Masseter Botox begins to wear off — repeat injection at month 4–6 if desired.
- Ongoing: Buccal fat result is permanent; masseter Botox maintained 1–2 times per year.
Who is suitable for the combination
Most candidates for buccal fat removal are also candidates for masseter Botox — they address different anatomical structures. Particularly good candidates for the combination:
- Patients with both a fuller mid-cheek and a visibly square lower jaw
- Patients with bruxism (teeth grinding) — masseter Botox treats both aesthetically and therapeutically
- Patients whose three-quarter view shows fullness at both heights of the face
- Patients who want a more visible result without aggressive buccal fat removal
Less suitable for the combination:
- Patients with already-thin masseter muscles
- Patients whose lower jaw is anatomically narrow (the buccal fat is the only contributor to lower-face fullness)
- Patients who don't want to maintain ongoing treatments

Is masseter Botox safe?
Yes — when administered correctly, masseter Botox is one of the safest aesthetic injectable procedures. The masseter is a well-mapped muscle and the injection sites are standardised.
Does masseter Botox affect chewing?
Minimally. There may be slight initial weakness chewing tough foods (steak, hard bread) for 1–2 weeks. Normal chewing is preserved.
How long does masseter Botox last?
3–6 months for the first treatment, often 6+ months for subsequent treatments as the muscle accumulates atrophy.
